REQUIRED FOR ALL WEIGHT LOSS PATIENTS

• Screening to determine if you qualify for medical weight loss, either with injectable semaglutide or another oral medication.

• If not, non-medication nutrition and lifestyle plan will be recommended.. 

• Basic lab review: recent labs within 6 months accepted. 

• Otherwise you will be given lab order  (to be preformed at lab locally). 

• Detailed history and review of symptoms. 

• Review of prior weight loss attempts. 

• Health coaching: goal setting. 

• Personalized nutrition plan. 

• Exercise schedule.

REQUIRED FOR ALL GUT HEALTH PATIENTS

• Basic lab review :recent labs within 6 months accepted. 

• Otherwise you will be given lab order (to be preformed locally).

• Detailed history and review of symptoms. 

• Review of health concerns : common examples fatigue, migraine, gastrointestinal problems, IBS, idiopathic hives, frequent colds, food sensitivities, anxiety. 

• Health coaching: goal setting/

• One accountability follow up visit is included. 

• Supplement recommendations (if appropriate)

• You will sign up for the new patient visit to include detailed review of symptoms, labs, and goal setting. 

• Plans include:

  • Cost of weight loss medication included for semaglutide plans only (oral medications if prescribed are typically affordable and insurance can be used to cover cost). 

  • Medication and supplies delivered to your home (semaglutide only).

  • Personalized nutrition plan. 

  • Exercise schedule. 

  • Biweekly coaching sessions. 

  • Follow up visits for medication adjustment.

• SILVER PROGRAM:​

  • Detailed review of symptoms. 

  • Advanced functional lab panel to assess metabolic and GI health. 

  • Lab interpretation. 

  • Nutrition plan based on test results. 

  • Supplement recommendations as appropriate. 

  • 2 follow up visits a month (every 2 weeks to assess progress).

• GOLD PROGRAM: 

  • This is designed for people who desire sustained lifestyle change and continuous personalized coaching.

  • The gold program includes all of what's included in the silver program plus weekly coaching.
